This Site and its Services have been developed in response to the FDA's requirement of healthcare providers to discuss with their patients the risks and benefits of treatment with Erythropoiesis Stimulating Agents (ESA) [Aranesp® (darbepoetin alfa), Epogen® (epoetin alfa), and Procrit® (epoetin alfa)]. In order to fulfill the requirement from the FDA to communicate the risk and benefits of these drugs to the healthcare providers, Companies have created this Site and its Services to educate and certify that the healthcare providers have conducted the appropriate training.

As described below, you may choose to provide Us with Personal Information through Our Site or Services. "Personal Information" includes any information that could be used to identify or contact you such as your name, address, telephone number or email address.
