This Site and its Services have been developed in response to the FDA's requirement of healthcare providers to discuss with their patients the risks and benefits of treatment with Erythropoiesis Stimulating Agents (ESA) [Aranesp® (darbepoetin alfa), Epogen® (epoetin alfa), and Procrit® (epoetin alfa)]. In order to fulfill the requirement from the FDA to communicate the risk and benefits of these drugs to the healthcare providers, Companies have created this Site and its Services to educate and certify that the healthcare providers have conducted the appropriate training.

PRODUCT REPORTS. If you use this Site or send Us email to report your experience with one of Our products, We may ask you for specific facts, including patient initials, age, gender, and prescribing physician. Federal regulations require Us to collect Personal Information from patients in order to file reports with the Federal Food and Drug Administration regarding the safety of Our products. If you are using this Site for reasons other than reporting a product experience, do not disclose Personal Information about a patient without valid patient authorization.
